    Medications and Plastic Surgery

    Preoperatively factors such as body weight fluctuation, intake of vitamins, Chinese herbs and other remedies are evaluated. It is important that you make your doctor aware of all medication herbs and other remedies you are taking whether or not you feel it is related to the surgery you are contemplating. A number of potentially harmful nutitional; restriction regimens and dietary supplements are currently quite popular.

    Patient should begin all purpose vitamin 2-4 weeks preoperatively . They need to begin taking 1000 mg of vitamin C twice daily, preferably longer 1-2 weeks preoperatively and two weeks postoperatively. In the immunocompromised  patient who was not pregnant some advise to take 25,000 IU of vitamin A daily for one week preoperatively and one week postoperatively to enhance the immune system.

    It has been reported that some patients taking herbs, Chinese medications and mega doses of vitamins have swelling and bruising greater then would normally be expected.

    Patients don’t always think that their herbal teas or vitamins as medications or even as supplements. A number of agents have been evaluated very carefully for their effects on bleeding. Among these is vitamin E; it effects platelet adhesion so it’s use should be discontinued before surgery. Those patients who are taking a multivitamin that contains 20 to 40 IU af vitamin E need not worry however. Those patients taking 200 to 400 IU daily should stop taking all vitamin E except for that found in there multivitamin. Patients should also discontinue garlic, which affects platelets function blood clotting. Patients who consume a lot of garlic, such as some ethnic groups, would probably need to decrease the amount of raw garlic in their diet.

    Fish oil has also been shown to affect bleeding. Some unusual teas, such as woodruff and Tonka bean, would need to be stopped because these agent can cause bleeding problems. Curcumin is actually a good anti inflammatory agent but it does have some anti platelet affects. In addition, someone taking penicillin, a cephalosporin or some psychiatric medication might experience anti platelet effects. Bleeding could be a problem is these were combined with those nutritional supplements mentioned so these patients should stop taking these supplements about one week preoperatively.

    Bromelain has been shown to resolve= experimental bruising more rapidly than groups not taking this preparation, and the patient would need to begin taking two days prior to surgery and as soon as possible after surgery for one week or until the bruising resolves. Bromelain comes from the pineapple plant and it reduces bruising and swelling significantly. It comes in capsules and can be found in health food stores and needs to be taken on an empty stomach.

    Another rather impressive agent is the homeopathic remedy arnica which helps to reduce soft tissue swelling and ecchymosis. A suggested regimen recommends taking30 cc of arnica two days preoperatively, 200 C one day preoperatively, another 200 C one help to one hour preoperatively and four doses of 200 C every four hours as soon as possible after surgery. No one knows precisely how arnica acts but it is known to affect blood vessels in some way that prevents or rapidly relieves bruising.

    It is important not to confuse homeopathic remedies with other medications-specifically herbs or Chinese medicines. The arnica homeopathic remedy is very different from the herbal agent arnica. The homeopathic remedy is a diluted and potenized form of arnica.

    There is no convincing evidence that topical agents such as aloe vera gel and vitamin E, improve scarring but they are probably not harmful as long as the wound is closed.

    As always, Dr Young attends many meetings and conferences with international doctors to enliven his practice.

    This week, he attended a Palomar meeting with Dr Maurice Addato, a Swiss Dermatologist, who is an expert on laser care.

    And last night, at the  Shangri-la Hotel, Dr Arthur Swift, a  Montreal Plastic Surgeon, spoke about the maths of beauty, and combination treatments.

    He discussed the 4 “D’s ” of aging- descent, deflation, degeneration, and disproportion. He showed how combination treatments with antiwrinkle injections, or neuromodulators, with volum+ can make huge improvements.

    “Bringing patients to their ideal beauty is our goal”, Dr Swift, so elloquently spoke about, and emphasized, phi, the golden ratio, and Marquardt’s face, which Dr Young has discussed in an earlier blog (Defining Beauty 2007).

    So some nice talks, meeting colleagues from overseas, always refining his techniques, which btw Dr Young has now perfected his latest technique with volum+!

    HK IMCAS 2010

    Just back from IMCAS, Hong Kong, Dr Young has a now an even bigger array of skills.

    Initially visiting the city of Guangzhou, to visit a few laser companies and clinics. He moved onto HK, where he attended his 2nd Asian IMCAS, which was held last year in Bangkok.

    Set in the HKSEC, overlooking the harbour, it is an amazing venue.

    Many interesting talks such as lip length reduction, fake anti wrinkle injections, mesotherapy, and filler lifting.

    Notable speakers were Michael Kane, Gordon Ma, and Benjamin Ascher.

    Incidentally Dr Young bumped into an old colleague, Dr Shiu, who now works in one of HK’s busiest cosmetic clinics.

    Over the next few weeks Dr Young will expound on the above and also discuss China.

    Dr Young was recently invited to talk on Latinos FM, 107.3.

    In the 2SER  studio in Broadway,  Dr Young joined Latinos FM Director Angel Montoya, and Sydney’s leading Salsa DJ Willie Sabor, for an hour he answered all questions about himself and Cosmetic Surgery.

    In the Latin community, plastic surgery has always been very popular, and this trend has been increasing with worldwide statistics.

    In worldwide statistics of Plastic Surgical procedures, USA leads by country, but in per capita terms, Switzerland and Spain are one and two. In a 2009 ASAPS paper on  Plastic Surgical  statistics in USA, there was a 16% rise within the Hispanic population’s seeking procedures.

    Brazil, Colombia, Mexico are some South American countries where it also is very popular.

    Angel informed all that many local people fly to Colombia to get treated.

    Popular procedures are rhinoplasty, liposuction and breast augmentation.

    In fact many Americans visit Colombia and Brazil for surgery and this has led to an aesthetic tourism boom over the last few years.

    But there is much more to Colombia than just cosmetic surgery or the history of Pablo Escobar. Colombia is a country with beautiful beaches and mountains,  with sparkling cities like Bogota/Medellin and Cali. There are ancient ruins, and  not forgetting it’s a diverse culinary delight.

    On the radio show, Dr Young dispelled some myths about Botox(e.g  made of poison, from cows etc), and elaborated on how it can help sweating, and boost a job application. Angel was even keen enough to undergo Botox treatment.

    He is now looking 5 years younger, and is keen for more improvements.

    One lucky listener won the call-in for 6 months of FFC anti-aging cream, this will be going out as soon as it is available.

    Of note, Dr Young attended the ISAPS conference in Rio De Janeiro in 2006, and  last year had the opportunity to visit the famous Barcelona Clinica Planas, headed by Dr Jorge Planas.

    Thailand, or Tyreland…

    I recently had a  trip to Bangkok for a short break, and visiting colleagues. Having been to Bangkok twice before, I was well and truly prepared, or was I?

    Well it was the red shirts vs the yellow vs the multicolored shirts?

    Thailand’s political climate had been simmering for months, then escalated into bloodshed in the week prior to me visiting Bangkok.

    Whilst there I really didn’t feel intimidated, coming from South Africa, we get a bit blase with this sort of thing, army and police all over the place, not to mention rolls of barbed wire.

    Only until the last day of my trip did the crisis make me a little uneasy, and this was when the army “entered’ the red shirt zone.

    Nevertheless, I did manage to do almost everything I wanted to do.

    Catching up with friends, and enjoying the excellent cuisine. Of note some great Bangkok restaurants- Sirocco, Koi, Kuppa, Bed Supperclub.

    A little shopping, tick.

    Tiger Temple, tick. Wow was that an amazing morning, feeding tigers, washing them, walking them, what beautiful cats.

    So intelligent, and tame. It is so great seeing them so loved and cared for, lets hope there is more and more tigers saved.

    The monks have done a wonderful job since the first tiger cub arrived in 1999, there are over 30 tigers now.

    This monastry was established in 1994 by the Abbot Phra Kanthitharo.

    Since its opening this place has gained a reputation as a wildlife sanctuary. It started with an injured jungle fowl given to the monks by the villagers.

    Then peacocks came attracted by the calls of the by then rather large colony of jungle fowl.

    An injured wild boar stumbled in to the monastery and the monks cared for him until he was released back into the forest.

    Now there are four species of deer, buffalo, cow, horses, and wild goats. All roam freely in these grounds.

    Ayutthaya, tick. The former Thai capital, was resplendent in the sun, with temples beaming. It was founded in 1350AD, by King Uthong. It was one of the richest cities in Asia by the 1600’s, but sadly by 1767 most of the city was burned by the war with the Burmese.

    One of the most famous sites is of  a buddhatighead of Buddha, wrapped within the roots of a fig tree. Simply stunning. These ruins show various styles of architecture.

    Back to Bangkok.

    So as the turmoil settles, and the 30 or so fires that were set smolder, I hope my friends in Bangkok stay safe, and the smiles return to this amazing city.

    Travel to learn, and grow as a human.

    Rosemount Australian Fashion Week begins

    Dr Young has been invited to the Sydney shows, and will tried to make most of the night shows.

    Opening night he saw Aurielia Costarella , and Bec and Bridge at the OPT, then in Alexandria it was Stolen Girlfriends Club.

    Closing night, Ksubi, was without animals.

    Beauty comes in all shapes and forms. From the hairstyles, to new makeup uses, painted on the body where spashes of paint.

    Big cheeks where in, as was intricate eye makeup.

    Backstage Mr Perdis, was working his magic for Alex Perry.

    30 skin care myths

    1. Facial exercises tone facial muscles and make a person appear younger. The face is the only part of the body where muscles are attached directly to the skin; there are no facial ligaments and tissue. Constant facial exercise and tugging contribute to additional lines. Actually, wrinkles often form along expression lines caused by facial movements.
    2. Vitamin E minimizes scarring.Vitamin E is an antioxidant and helps build skin, but there is no evidence that it does anything to help with scarring. There is even some research that suggests it may have a negative effect on scarring.
    3. Cucumbers help reduce puffiness around the eyes.The principle ingredient in cucumbers is 90% water with the balance being inert fiber. They can be soothing and, with moisture, hydrate skin temporarily. The same results can be obtained from a cold compress.
    4. Skin pores open and close.Pores are openings in the skin that allow oils (sebum) to reach the surface. If pores are larger, this can be due to dead cells, genetics or scarring from squeezing blemishes. Possible treatment- IPL/awi/rolling.
    5. The higher the skin protection factor (SPF) rating, the better.SPF ratings, soon to be revised by the U.S. Food and Drug Administration (FDA), only refer to protection from UVB rays. A person needs sun protection that has chemical and physical blockers, plus antioxidants. A higher SPF also gives a false sense of security and introduces more potentially harmful chemicals to the body. Plus, an SPF of 50 is only marginally more protective than an SPF of 15; an SPF of 30 has only 2% more protection than an SPF of 15, and a 40 has only 1% more than a 30. Sunscreens need to be reapplied every 90–120 minutes.
    6. Layering several products with SPF ratings increases protection.You are only protected to the extent of the higher rating of one product. A foundation with an SPF of 10, moisturizer with an SPF of 15 and a sunscreen with an SPF of 20 does not yield an SPF rating of 45.
    7. Topical creams containing collagen can replace collagen.There is a lack of impartial, empirical evidence that the topical application of collagen or elastin can penetrate the dermis, even when using nanotechnology. They can provide moisturization to the epidermis, but only injections are conclusively effective.
    8. Mineral oil is bad for your skin.Today’s cosmeceutical mineral oil is a far cry from the industrial type that was previously used on skin. It is a very effective ingredient in helping remove excess oil from the skin. Oil attracts oil, and the modern mineral oil formulated for use in skin care has a different molecular weight and will not harm skin or clog pores. Mixed with kaolin—fine clay—it makes a potent cocktail to assist in controlling oil production in problematic skin. Vitamin A is the best ingredient to normalize skin.
      • 9.Mineral oil is comedogenic.Cosmeceutical-grade mineral oil is not comedogenic. The myth is that industrial-grade mineral oil and lubricants are the same as those used in cosmetic ingredients. So-called medicinal white mineral oil has met with stringent safety standards. To remove sebum plugs in the follicles, an oil-based substance is needed. Water-based products cannot melt or remove sebum plugs.
      • 10.Preservatives in skin care products are bad.Preservatives help prevent the growth of bacteria, fungi and other organisms that can not only deteriorate a product’s effectiveness and spoil the product itself, but also allow harmful bacteria to get on or in the skin. Although there is concern about the use of parabens, the research is not definitive that topical application leads to harmful accumulations. Parabens are found throughout nature; for example, many fruits and vegetables, such as strawberries, are naturally full of parabens. But further study is needed.
      • 11.Packaging is not important.Packaging in skin care is vitally important—not for aesthetic reasons—but to protect the efficacy of the ingredients. Wide-mouth jars, transparent containers and pumps that are not airless all pose problems in keeping ingredients safe and potent.
      • 12.Chocolate and greasy foods cause acne.Eating chocolate does not cause acne. Hormonal factors, bacteria and skin cells are at the root of problematic skin, and stress can exacerbate flareups. Some individuals may have allergic reactions to foods that can cause inflammation, as well.
      • 13.Natural and organic products are always better.Buyer, beware! Many natural and organic products are not as they claim. Plus, many times, active ingredients have to be synthesized to be bioavailable and efficacious. Synthetic compounds can actually be identical to those found in nature and be more effective. Natural vs. laboratory-processed should not lead to an up or down decision about whether a product is good or bad. Not all chemicals are bad, and not all natural or organic ingredients are good.
      • 14.Using larger quantities of a product will yield better results.Less is more. Normally, a pea-sized amount of facial product will do the trick. Excessive amounts can cause skin problems and waste money.
      • 15.Blackheads are a caused by improperly cleansed skin.Blackheads or comedones are caused by clogged pores, and excessive scrubbing can irritate and further inflame skin. Blackheads often contain dirt, oil, and dry and dead skin cells that need to be removed. Products that help dissolve sebum are the most effective.
      • 16.Drying problematic or oily skin clears up acne.The opposite is true. When skin becomes overly dry, an environment is created in which the skin is signaled that is too dry and produces more oil. Use drying products sparingly, and look to lightly moisture oily skin. The goal is to keep skin balanced.
      • 17.Get a base tan to prevent burning before going on vacation.Any tan is a scar, and there is no such thing as a healthy, safe tan. Self-tanners are the safest way to obtain tanned skin.
      • 18.All sun damage to skin occurs before 18 years of age.Sun damage continues to occur throughout life, although recent information suggests less than 50% of sun damage happens before a person is 18. It is never too late to protect your skin.
        • 19.Indoor tanning is safe.The argument that tanning beds and booths do not cause skin mutations that may cause cancerous lesions to develop is patently false. UVA rays found in indoor tanning lead to deeper, more harmful skin damage. You do not have to have a sunburn to create damage to skin cells.
        • 20.Antioxidants reverse wrinkles.Antioxidants are essential in fighting free radical formation and are important in helping prevent skin damage, but they cannot make wrinkles go away.
        • 21.Skin damage and signs of aging can be cleared up quickly.If a product sounds too good to be true, you can bet its claims are false. The damage did not happen overnight, and it cannot be magically repaired. Expect at least three skin cycles—a cycle can be between 21–40 days, depending on age—to begin to see measurable results.
        • 22.All alcohol in skin products is bad.Some compounds that contain alcohol can act as emollients, which can decrease the skin’s water loss. Cetyl, benzyl and oleyl alcohol are examples of good alcohols. It is important to know what comes before the OH in chemical compounds.
        • 23.Sun exposure will improve acne.Yes, sun exposure can hide the appearance of acne for awhile, but will lead to skin damage, pigmentation and drying that signals the skin to produce more oil.
        • 24.Alcohol abuse can cause your nose to become red and bulbous.The intake of alcohol can temporarily dilate blood vessels and make skin appear flushed, but in most cases, a large, inflamed, red and bulbous nose is a result of rosacea.
        • 25.Skin repair only happens at night.A good night’s sleep is certainly helpful to skin health; however, skin repair is ongoing. Inadequate sleep can cause stress, skin puffiness and can slow the natural development of collagen.
        • 26.Skin care products can last three or more years.Despite a number of claims to the contrary, most skin care products lose a great deal of their potency within 12 months. It is best to use the entire contents within one year because preservatives do not last forever and ingredients can get contaminated with bacteria, or they can evaporate.
        • 27.Strong scrubs, soaps and abrasives are good for your skin.Be careful how you wash your face. Too much scrubbing or too many abrasive products can remove protective oils, create tiny micro tears and contribute to aging, irritated skin. Less is more, and a gentle cleanser and light moisturizer work well for most people.
        • 28.Vitamin A thins the skin.Actually, the reverse is true. Skin can become thin due to the lack of vitamin A because it helps to create new, healthy and normal skin cells. Vitamin A is arguably the most important skin care ingredient, bar none. It is one of the few—if not the only—ingredient that is backed by more than 50 years of objective, scientific research supporting its efficacy.  We sell our exclusive Vit A cream.
          • 29.The only form of vitamin C that works is L-ascorbic acid. L-ascorbic acid only remains in its most potent state for a limited time. A new era in vitamin C formulations, one of the best antioxidants for your skin, has arrived. There are several forms that have been developed that are not water-based, which means they can better penetrate the skin and remain more potent for longer periods of time.
          • 30.There is one antioxidant ingredient that is the best. Every year, there is a hot, newly discovered antioxidant that is touted as the best, but this is not true. A cocktail of antioxidants provides better results than just one. Seek products with a range of antioxidants.

    Increased wrinkles around your mouth or a shortened lifespan of your injectable fillers,  consider whether gum-chewing, it  may be the culprit.

    It freshens your breath and helps you quit smoking, but some cosmetic surgeons believe chewing gum does one more thing: It gives you wrinkles.

    “Many of my patients who are gum chewers have a certain pattern of wrinkles around their mouths,” says Joel Schlessinger, MD, a board-certified dermatologist and cosmetic surgeon from Omaha, Nebraska. “And I think the gum is responsible to some degree for it.”

    Although no studies have been done showing a link between chewing gum and wrinkles, the topic does come up with some regularity on beauty blogs. Experts attribute the gum-wrinkle connection to two things. First, there’s the repetitive motion of chewing that causes lines and folds around the mouth due to muscle overuse, says Hema Sundaram, MD, a Washington, D.C.-area cosmetic surgeon and laser expert. “I believe chewing gum promotes muscle overactivity and potentially breaks down support tissue within the skin, contributing to volume loss and perhaps loss of skin elasticity,” she says.

    What’s more, chewing gum can dislodge dermal fillers that people have injected into their faces to plump up their wrinkles. “It makes your dermal filler last a shorter period of time,” says Sundaram.

    Schlessinger says he’s observed this with his patients, as well. “The act of chewing gum can dislodge the fillers earlier,” he says. “It actually pushes them out of the area. They dissipate a lot quicker in gum-chewers, in my opinion.”

    Certainly limit smoking, using straws, and whistling!

    Cosmetic Surgery not taboo anymore

    Spas and medical spas often work with clients who are considering plastic surgery, and it is good to know that it isn’t as taboo as it used to be. Sharing this information with clients who are considering plastic surgery may help them to feel more comfortable about their decision.

    Cosmetic surgery is everywhere these days: TV, magazines, Internet and more. And according to new data, society is indeed more comfortable talking about cosmetic surgery than ever before.

    In a survey released today by the American Academy of Cosmetic Surgery (AACS), 71% of respondents think society is less judgmental about cosmetic surgery than it was just five years ago. Furthermore, about 62% said society’s attitude toward cosmetic surgery made them feel more comfortable about getting a procedure.

    “This is a confirmation of what many of us have felt for a long time; that cosmetic surgery has become mainstream,” said Dr. Mark Berman, AACS president-elect. “A variety of factors have contributed to the normalization of our industry, but the bottom line is that people are not afraid to talk openly about cosmetic surgery anymore.”

    According to the survey, the most accepted invasive procedures are breast augmentation (42.4%), face lift (32.2%) and tummy tuck (32.1%). Among noninvasive procedures, the most accepted are laser hair removal (61.9%) and Botox (61.6%). Additionally, 52% of respondents said they would tell family about having invasive cosmetic surgery.

    About 20% said they would consider getting cosmetic surgery in the future, while about 22% were unsure if they would. Respondents under the age of 40 were nearly twice as likely to consider having a procedure in the future.

    “It’s been a long road but thanks to the hard work of so many, cosmetic surgery is rightfully gaining acceptance in our society,” said Dr. Patrick McMenamin, AACS president. “We’ve reached a point where people are no longer quiet about having a cosmetic procedure and that is important to all of us.”
